Join Our Team! – Care Coordinator Upper Valley Services has a long-standing history of providing high quality, community-based, person centered supports to Vermonters. We are searching for the right person to join our efforts of keeping Vermonters, who require nursing home level of support, in the community. As a Care Coordinator, the person will be responsible for, interfacing with local hospitals, nursing homes and other care partners, and providing on-going service coordination for participants who have transitioned from a medical facility to a long-term shared living home. The ideal candidate will share our values of community based care, hold a Bachelors’ Degree in a related field, have a strong background in care management.
